Preschool Developmental Screening

Dear Parents/Caregivers,

The Grafton School District offers a comprehensive and confidential screening tool to help guide and keep track of your child’s growth and development between the ages of 3 and 5. We encourage all preschool aged children to be screened.

Children develop skills and reach milestones at their own pace. How is your child doing? We encourage you to take 10–20 minutes to check with the Ages & Stages Questionnaires®, Third EditionTM (ASQ-3). Based on your responses, your results will help you see if your child’s developmental progress is on time and alert you to any potential concerns. Your questionnaire will be scored and you will be contacted to discuss the results and any suggested activities you can do at home. If there are concerns with the screening results, families will be offered a free comprehensive evaluation to determine if additional services are warranted through the school district. Evidence shows that early intervention is key and the sooner you address a developmental delay or special need, the better chances your child has of long-term success.

If you are interested in receiving a screening questionnaire, please call Aly Neumyer at 262-376-5717 or email Aly at . A questionnaire can be sent electronically, mailed to you, sent home with another child, or you can pick one up from the District Office.
